Treatment Model

TM  106.01  Provider must submit proof that all staff and caregivers have completed the initial training on the Treatment Model.

TM106.02    Provider must submit the curriculum that will be used to train Staff and Caregivers initially on the Treatment Model, as well as the curriculum for pre-service and annual training.

TM 106.03    Provide a detailed description of the methods used to educate and ensure awareness of the Treatment Model by children/youth/young adults in the provider’s care.

Logic Model

LM  107.01    specific to the service package or Add-On Service, provide a graphic illustration of the program’s logic model in accordance with requirements defined in the “Commonly Used Terms” section of the T3C  System Blueprint.  The graphic illustration of the Logic Model must demonstrate integration of the Treatment Model in the program.
