TM 101 The organization has a policy that describes the treatment/program model being utilized. Children, youth, and young adults must be aware of, and all staff and caregivers providing these services must be trained in, practice, and remain current with, delivery of the treatment/program model

TM  102  Provider must submit proof that all staff and caregivers have completed the initial training on the Treatment Model.

TM103    Provider must submit the curriculum that will be used to train Staff and Caregivers initially on the Treatment Model, as well as the curriculum for pre-service and annual training.

TM 104  Provide a detailed description of the methods used to educate and ensure awareness of the Treatment Model by children/youth/young adults in the provider’s care.

TM 105 Is the treatment/program model evidenced based or informed, and how does this evidence support and address the specific needs of the population served?

TM 106 Who is responsible for the on-going development process of the treatment/program model?

TM 107 Is the model a Trauma Informed Model?

TM 108 Do Policies and Procedures describe how the Treatment/Program Model is designed for the specific population being served?

TM 109   Ensure the integration of the treatment model into policies and procedures relevant to the specific  Service Package.

Logic Model

LM 110 Has the provider developed a graphic depiction of the Logic Model?

LM 111 Does the Logic Model address the following key components:  Input, Activities (to include trauma informed), Outputs and Outcomes (to include process of ongoing evaluation of the Logic Model’s effectiveness)?

LM 112  Does the Logic Model define/describe the provider’s specific service package(s)?

LM  113  Specific to each service package or Add-On Service, provide a graphic illustration of the program’s logic model which demonstrate integration of the Treatment Model in the program.
